B Preface Thank you for choosing the iConnect Handheld Device system. iConnect Handheld Device is a handheld scanner used to assist the nurse in the medication administration process. iConnect Handheld Device promotes nurse and patient safety by verifying that the right patient is getting the right medication, at the right dosage, at the right time, via the right route. The iConnect Handheld Device focuses on the daily use, minor maintenance, and troubleshooting of the iConnect Handheld Device and the iConnect DOT Manager. B Chapter 1 System Overview Introduction iConnect System Definition The iConnect System, created by intelliDOT Corporation, is a wireless handheld workflow tool that allows patient safety checks, documentation, and communication functions to be completed quickly and easily by a nurse at the bedside. The iConnect System was custom-designed with nursing professionals to provide state-of-the-art patient safety while improving overall nurse productivity and efficiency. The system helps nurses manage a myriad of tasks with a single handheld wireless device that is linked in real time to the hospital's clinical information system. The heart of the IntelliDOT System is the iConnect Handheld. Using three simple keys, nurses can complete a variety of tasks and commands. With just a quick scan of a bar code or iDOT, critical information can be accessed. Information is delivered right into the caregivers hands, right when it is needed. iConnect System Benefits Studies performed in acute care hospitals show that bar code-based medication error prevention systems greatly reduce medication errors that occur during the medication administration process. These errors have caused regulatory agencies and some state legislators to mandate their use by 2005. intelliDOT iConnect System User Guide Confidential 1-1 iConnect System Components P/N TP0001-01 Rev. B Despite the existence of several bar code checking products, as of mid-2004 less than five percent of the 5,800 acute care hospitals in the U.S. have purchased bedside error prevention systems. Why? Poor product usability, prohibitive cost, and difficult installation and integration with existing hospital systems. The iConnect System is an easy-to-use, easy-to-integrate solution that is priced significantly less than other bar code medication safety systems. Built from the caregiver's point of view, the system combines a unique handheld device with innovative Documentation of Treatment (iDOT) symbology, to allow nurses to complete documentation at the bedside with just one hand. iConnect Handheld iConnect System Manager iConnect Server iConnect Handheld The iConnect Handheld is a small, battery-operated wireless device that incorpo-
rates two types of scanning technologya regular bar code scanner and a special digital camera-based scanner that reads a small information-rich symbol that intelliDOT calls the iDOT. The bar code scanner reads all standard bar codes used throughout the medication administration process-
from medication packaging (both in-house and manufacturers' bar codes), to patient wristbands and nurse badges. The iConnect Handheld scanning device reads the unique iDOT symbol to accomplish a wide variety of documentation and communication functions. 1-2 Confidential intelliDOT iConnect System User Guide P/N TP0001-01 Rev. B iConnect System Components Physical Description Weighing less than 8 ounces, the iConnect Handheld can be used comfortably in either the user's left or right hand. It has an extremely simple interface composed of:
A message display window from which the user views confirmations, instructions, and other information Three buttons that allow the user to scan a bar code, scan an iDOT, or confirm an action or instruction A visual and audible alert system to announce important information, such as critical overdue medications or new abnormal lab results The iConnect Handheld communicates continuously with the hospital computer systems using a wireless, radio frequency (RF) network operating on a standard 802.11(b) wireless protocol. Wireless access points owned by the hospital provide the iConnect System with an information channel to send and receive information from the iConnect Server, iConnect System Manager, and iConnect Handheld. Charging the Device A rechargeable battery powers the iConnect Handheld. When not in use, place the iConnect Handheld unit in the charger. It takes approximately 4 hours to completely recharge a drained unit. Each time the iConnect Handheld is placed in the charger, it will perform a wireless software maintenance routine. Maintenance may take anywhere from 30 seconds to several minutes. Do not remove the iConnect Handheld from the charger during software maintenance. Once maintenance functions are completed, system charging will begin. Operating the iConnect Handheld Caregivers use the iConnect Handheld to scan the bar code on the nurse's name badge, the patient's wristband, and the medication in order to provide verification of the correct patient, medication, dosage, time, and routethe Five Rights of medication administration. The iConnect System provides nurses with a way to quickly and easily check medication accuracy and enter patient data at the bedside, while receiving real-
time clinical alerts and reminders as well as other information anywhere in the their work area. intelliDOT iConnect System User Guide Confidential 1-3 iConnect System Components P/N TP0001-01 Rev. B Durability The iConnect Handheld is a sturdy piece of equipment designed to stand up well to the continuous demands of nurses working in busy patient care areas. While it is both water- and dust-resistant, the iConnect Handheld should not be submerged or exposed to significant moisture for any extended period. In addition, even though the device is rugged, care should also be taken to avoid dropping it or subjecting it to significant or repetitive impacts. The iConnect Handheld operates within the normal range of indoor temperatures, but it should be protected from any temperature extremes. Using the iConnect Handheld In addition to using the iConnect Handheld as a bar code scanner, the iConnect System provides users with a flexible tool to streamline the workflow process. intelliDOT's patented Documentation of Treatment (iDOT) system allows hospitals to create their own customized documentation and communication applications that can be accessed by scanning a conveniently placed small, unique dot-like symbol. Using the iConnect Handheld to scan iDOTs, medication administration and other forms documentation flows wirelessly into an electronic database while the nurse is at the patient's bedside. The iDOT is the foundation of intelliDOT's Documentation of Treatment system. The iDOT symbology is built on a patented, proprietary fingerprint-like symbol that is only 5 millimeters in diameter and can hold a vast amount of information. Over 19 billion, billion combinations are possible providing a virtually unlimited supply of unique iDOTs for any hospital. The iDOT symbol is easy to read with the iConnect Handheldits unique design allows it to be scanned upside down, sideways, or in any orientation. And a portion of the iDOTs large storage capacity is dedicated to forward error correction, allowing it to be accurately read even when portions are damaged. The iDOT provides nurses with a shorthand scanning system to record information quickly and accurately and to issue commands with a quick scan using the iConnect Handheld wireless reader. Tasks that can be accomplished with this system include:

B iConnect System Components iConnect System Manager Physical Description The iConnect System Manager is a client-server application that operates on the hospital's local area network (LAN). Access / Operation of the iConnect System Manager The application may be accessed via any assigned PC-based network client. Typically, a PC on each nursing unit will host the iConnect System Manager software program. Functions The iConnect System Manager contains all configuration settings for the iConnect System. Nurses can use the iConnect System Manager to look up or edit medication data, make patient assignments, and print reports. Pharmacy computers are also clients to the iConnect System Manager application. Pharmacy will routinely access the iConnect System Manager for formulary, database, and system maintenance and reports. iConnect Server Physical Description The iConnect Server is an enterprise-class server that operates on the hospital network. The iConnect Server incorporates an HL7 interface engine in order to link hospital information systems to the iConnect Handhelds over the hospital's network, through wireless access points located in patient care areas. Purpose The iConnect Server interfaces with a variety of hospital information systems to communicate both clinical and administrative information to and from users in patient care areas. Connecting to the pharmacy information system provides real-
time physician order updates. Interfacing to the hospital's Admissions, Discharge and Transfer (ADT) system, the iConnect Server provides patient location and demographic information. Other optional interfaces provide links to the laboratory information system and hospital electronic medical record systems to integrate documentation into the master patient file. intelliDOT iConnect System User Guide Confidential 1-5 iConnect System Installation P/N TP0001-01 Rev. B iConnect System Installation The iConnect System installation consists of the following key activities:
Interface installation Wireless access point installation Bar coding (nurse badges/patient wristbands) Pharmacy system setup Nursing workflow setup Wireless Access Point Installation Function, Location, and Definition The iConnect System uses 802.11b as a wireless communication protocol because of its stability, availability, and range. Wireless access points must be installed in all areas where iConnect Handhelds are used within the hospital. Typically, three to four access points need to be installed for each nursing unit. Installation Responsibility Since wireless access points are often used to support multiple applications, it is the customer's responsibility to assure installation in the hospital facility. When needed, IntelliDOT may suggest third party vendors or other assistance in completing the installation. Purpose Each handheld device must be able to communicate continuously with the iConnect System Manager in order to maintain the real-time flow of information to and from each nurse. Interface Installation Definition The iConnect System utilizes an HL7, TCP/IP, port-based, healthcare data exchange interface. Information Components The iConnect Server requires an interface which transmits verified pharmacy orders; ADT administrative information; and formulary updates, when appropriate. In addition, the iConnect can send outbound medication administration documentation information (i.e., charting messages) to the clinical system. 1-6 Confidential intelliDOT iConnect System User Guide P/N TP0001-01 Rev. B iConnect System Installation Installation Responsibility The intelliDOT team of engineers works with the hospital to configure and install the necessary system interfaces. Development and Testing As part of the installation process, the intelliDOT team will provide documented verification that all identified interfaces to the iConnect Server perform according to their specifications. The intelliDOT installation includes a test system environment for quality testing of system functionality based on received HL7-
formatted messaging. Bar Coding The iConnect Handheld incorporates a scanner to check bar codes for the medication administration process. In order to implement the iConnect System, the hospital must have bar codes on nurse badges, patient wristbands, and medication packages. intelliDOT works with hospital management to identify and install a suitable badge, wristband, and medication bar code solutions. Nurse Badges The iConnect System requires that each user scan a unique user identification bar code (typically found on the nurse's name badge) to log in to the system and to close out each medication administration process. This provides security and accountability for all functions performed by the nurse with the iConnect Handheld. Patient Wristbands Patient wristbands with bar codes are required in order to match the correct medications and information to the right patient. Each patient must wear a bar-
coded wristband that can be read using the iConnect Handheld. Medication Packages The iConnect System is designed to work with the unit-dose medication distribution system. Medications not supplied in commercially produced unit-
dose packages are repackaged in the pharmacy, either through the use of a medication packaging system or by a pharmacy technician providing manual bar code labeling. A pharmacy technician can be assigned to perform both of these functions under the supervision of a pharmacist utilizing a "pharmacist check tech" quality assurance process. B Pharmacy System Setup The pharmacy information system provides the information needed for the iConnect System. Physician orders are reviewed by the pharmacist, entered into the computer, and medications are prepared and dispensed to the nursing unit. Once completed, the physician order is sent through an electronic link between the hospital pharmacy computer and the iConnect Server. For orders to be correctly interpreted by the iConnect System, hospital-specific clinical settings, warning parameters, and sig codes are set up for each formulary drug item. Formulary Because the iConnect Handheld is designed to scan bar codes on medications at the patient bedside, the hospital pharmacy department is responsible for bar-
coding all medications, both during system setup and on an ongoing basis. The intelliDOT installation team works closely with the pharmacy to set up this process. Often, several medication brands may satisfy an order for a particular medication item. The iConnect System database is organized to allow multiple medications
(similar generic) to satisfy an order, if scanned at the bedside. This database arrangement allows the iConnect System to properly identify medications when scanned and to conduct proper checks for the Five Rights of medication administration. System Configuration Process The steps for creating the hospital-specific iConnect System Manager medication database on site include:
1. The hospital downloads the hospital medication list including medication description, MedID, and NDC (if available) from the pharmacy information system into the iConnect Server. 2. The iConnect Server organizes the data into a defined format and imports it into the iConnect System Manager formulary table. 3. Using the iConnect Handheld, intelliDOT technicians working with hospital staff scaning package bar codes for each formulary database line item. If the medication cannot be scanned, information must be manually entered from the product package. 4. Clinical Settings The pharmacy is responsible for ensuring that clinical settings, warning parameters, and sig codes are set up for each formulary drug item, as necessary. B Charging the iConnect Handheld Charging the iConnect Handheld The iConnect Handheld is powered by rechargeable batteries and should be stored in the charger when not in use. Recharging time is approximately 4 hours when battery power is near zero. While in the charger, the iConnect Handheld will also perform wireless software maintenance. Software maintenance requires 30 seconds to several minutes to complete. Once fully charged, battery power will last for approximately 12 hours. When the charge falls below 18%, the low battery warning will appear. If this occurs, return the iConnect Handheld to the charging cradle, and obtain a fully charged replacement. 1. Always verify that the iConnect Handheld is inserted correctly into the power cradle to ensure that the battery will recharge properly. 2. The iConnect Handheld will display Updating Software when software maintenance is in progress. Caution: Do not remove the iConnect Handheld from the charging cradle during software maintenance. 3. Before removing an iConnect Handheld from the charger for use, verify that the device is adequately charged by observing the percentage (%) of charge displayed on the screen. Bar codes are best scanned by holding the bar code scanner 6-8 inches away and pressing one of the bar code buttons to allow the laser to read across the bar code. iDOTs are scanned by placing the clear cone or the DOT scanner directly on the page, over the iDOT, and pressing the DOT scan button. Scanning Bar Codes 1. Hold the iConnect Handheld perpendicular to bar code lines and press the Bar Code Scan button on the right side of the iConnect Handheld. A red laser will be beamed from the bottom right front of the iConnect Handheld. 2. Point the beam at the bar code. 3. 4. If a up-tone chirp is heard, the scan is successful. If a down-tone chirp is heard, repeat steps 1 and 2. Note: For best results when scanning bar codes, hold the scanner at a 30oto 40o angle, approximately 6 to 8 inches away from the bar code. Omitting a Due Dose With iConnect, every dose of medication must be accounted for by the system and the nurse user. Based on clinical judgement, you may decide to permanently skip a medication dose that is due. iConnect refers to this as "omitting" the scheduled dose. An omitted dose will be charted on the electronic medication administration record as omitted, or not given, with full documentation of the date, time, caregiver who omitted the dose, and the reason for omitting. Note: Remember, omitting a dose that is not administered is important to iConnect because the system will indefinitely continue to assume that the next dose of the due medication that is given is the first due or overdue dose. Without properly omitting a due or overdue dose that is purposely not administered, the accountability and documentation for future doses given may be inaccurate. To omit a due or overdue dose, perform the following steps when the Dose Reminder or Overdue Dose Alert:

To do this, simply press OK when the message is displayed. The message will appear again in ten minutes or the next time you log in, whichever comes first. This process can be repeated if necessary. The iConnect electronic medication administration record does not capture the snoozing of a Dose Due Reminder or Overdue Dose Alert. Repeated snoozing may result in giving and documenting a scheduled dose late. If you decide not to give a scheduled dose, be sure to Omit the dose rather than snooze it. (See Omitting a Due Dose). B Dose-Due Reminders and Overdue Alerts Using Cover Patients to Transfer Patient Responsibility When you are beginning a shift, the responsibility for patient medication care must be transferred from the off-going nurse to you. During a shift, it may also be necessary for a nurse to accept responsibility for another nurse's patients while that nurse is temporarily away from the patient care area. The iConnect Handheld can easily transfer patient medication responsibility, including medication reminders and alerts, from one nurse to another using the Cover Patients feature. The following steps should be followed by the nurse (e.g., on-coming) who is accepting the responsibility for patient medication coverage:

B Medication Administration Warnings The iConnect System will alert the nurse to certain situations that may exist at the time of medication administration. In cases where it is necessary to bypass the alert, the system may ask the nurse to provide a reason or to enter other patient information. This is easily accomplished by scanning iDOTs. The following types of warnings are provided:
Early or Late Warnings Early or Late Warnings may display when a medication that the nurse is scanning prior to administration is early or late. This is determined by either the scheduled time or the PRN interval order for the medication order's and other timing parameters set in your iConnect system based on hospital policy. If the nurse decides to give a medication despite an early or late medication warning, the iConnect Handheld may be configured to prompt for entry of an override reason to document why the warning is being bypassed. This information will be recorded in association with the dose, which results in a more complete and accurate medication administration record for the patient. Clinical Warnings The iConnect System may be configured according to hospital policy to display helpful information for the nurse when a medication is scanned. These "Clinical Warnings" may include messages such as "Do Not Crush", "Give with Food", or
"Dilute Before Use". The nurse acknowledges the warning simply by pressing OK to continue the medication administration process. Clinical warnings are for the caregiver's benefit only and are not recorded in the medication administration record. Timeout Warnings If a user is logged on to the iConnect Handheld and it senses no activity (such as a scan or a keypress) for 5 minutes, an audible and visual alert will sound/appear. The visual light for yellow is a reminder, red is an action and green is charging state for the iConnect Handhelds. If keypress is not recorded after 10 minutes, the iConnect Handheld will automatically log off the user and return to the home screen. The timeout warnings only appear in the middle of a medication pass. Note: It is important to log off when the iConnect Handheld is not in frequent use. If an automatic logoff occurs, transaction data may be lost. For example, if medications were administered without a final scan of the user's badge to save the data, if a subsequent automatic logoff access the transactions will not be recorded. 2-22 Confidential intelliDOT iConnect System User Guide P/N TP0001-01 Rev. B Medication Administration Warnings Unknown Medication The pharmacy system manager maintains the iConnect medication database, including information for the identification of all formulary medication bar codes. However, if a dispensed medication bar code is scanned and not recognized by the iConnect system, the unit will display "Requesting Information" followed by the message "Unknown Medication". If this should occur, contact the pharmacy as soon as possible to let them know the medication in question. Typically, the medication and its associated bar code will need to be added to the intelliDOT medication database. Dose Out of Range Based on hospital policy, some medications in the iConnect System may be configured to require that the entry of a dose given amount be with a defined range. For example:

For medications requesting the nurse to enter a dose, the iConnect Handheld will display out of range if the dose entered is more or less than the accepted dosage range set by pharmacy. If this message is displayed, the nurse must reenter a dose in range, or scan a reason for giving the out of range dose. If the nurse decides to proceed with giving a dose amount that is outside of the range defined in the iConnect System, an Override Reason iDOT may be scanned when the out of range warming is displayed. intelliDOT iConnect System User Guide Confidential 2-23 Medication Administration Warnings P/N TP0001-01 Rev. B No Order Found Warning The No Order Found warning will appear when a patient is scanned followed by the scanning of a medication for which no current order exists in the iConnect system or pharmacy system. This can happen when:
1. You are scanning a medication for administration before pharmacy has processed and entered a new medication order. In this case, you should double check the patient, medication, dose, route, and administration time (the 5-
Rights). If you decide to proceed after receiving the No Order Found warning, you will be prompted to enter an Override Reason. Override Reasons are configurable by your system manager. iDOTs for Override Reasons can be found on the Medication Administration DOT sheet. Warning: Always check the patient chart and Physician Order if iConnect Handheld prompts with No Order Found. It is possible that the medication may not be ordered for this patient. Battery Low Warning The normal battery life for the iConnect Handheld is approximately 12 hours. The battery low warning appears on your iConnect Handheld LCD when the battery charge falls to 18%. The battery icon located on the bottom left side of your iConnect Handheld LCD shows the amount of battery life remaining. Be sure to check this periodically throughout the day to assure that you have enough battery life to get through your shift. Refer to the following for an illustration of the battery icon on the iConnect Handheld:

There are five different cases where the special preparation work path applies, and there are two possible processes for administering these doses once prepared. B MAR Notes The IntelliDOT system provides you a way to create patient specific notes on the Patient Edit screen, the Order Detail screen, and on the Administration Specific notes on the On-Line MAR on the iConnect System Manager. Patient Specific Notes The Patient Note field is able to handle up to 120 characters on the patient page of the iConnect System Manager. The last edit date, time, and user are shown at the end of the note and the iConnect System Manager keeps a history of changes. During an edit, a user can either modify or delete an existing note. To delete a note, a Clear Note button is provided that clears the note field. If a Patient Note is deleted, the field on the report will again be blank, but the deletion date/time/user will be logged. The user may also edit the previous note if desired, or enter an entirely new note by highlighting and erasing the previous note, then typing a new one. No changes are saved until the user clicks Save. This marks the previous note as deleted and, if applicable, creates a new note, both of which capture the date, time, and user. The field label on reports should be Patient Note:. Adding Patient Specific Notes To add a note to the patient edit screen, perform the following:
1. After logging into the iConnect System Manager, select Patients. The Patient screen will appear. 2. Click in the Patient Note field and enter your patient note. B Medication Order Specific Notes This Order Note is specific to a medication order (i.e patient likes to take this med with applesauce). The Order Note is a 90-character field that follows the same entry/edit process as the Patient Note, but it applies to an individual order. To enter an Order Note, perform the following:
1. After logging into the iConnect System Manager, select Patients. The Patient screen will appear. 2. Click on a Patient. 3. On the Patients screen in the Orders window section, double-click on a current Order to enter a note. The Order will be highlighted and an Order Detail screen will appear. 4. Enter the note in the Order Note field. This note will appear within the order on your MAR. 3-36 Confidential intelliDOT iConnect System User Guide P/N TP0001-01 Rev. B iConnect System Manager MAR Administration Specific Notes The administration is specific to a single administration event (i.e., patient vomited dose). The Dose Note can have up to 60 characters in the note field. The Dose Note that also follows the same entry/delete process except that it can only be edited or deleted by the person who created it. This note does not have to be entered by the person who gave the dose, but rather can be created by any user with system access and privileges to enter these notes for the specific patient. Note: These notes appear on the Medication Administration Record. To add Administration Specific Notes on the On-Line MAR, perform the following:
1. After logging into the iConnect System Manager, select Patients. The Patient screen will appear. 2. Click on a patient. 3. Click in the Orders window section. 4. Select the order to add the note. 5. Click the View MAR button. The On-Line MAR screen will appear. 6. Double-Click on the order or click on the Document Edit button 7. Click on the Administration Note field and enter your note. 8. Press OK. The Edit Administration window appears. 9. Enter your note information into the Administration Notes field. 10. Press OK. B Assigning Patients Assigning Patients via the iConnect System Manager is one of 2 ways to complete patient assignments. The other is using the iConnect Handheld. To assign patients via the iConnect System Manager, perform the following:
1. After logging into the iConnect System Manager, select Patient Assignment. 2. Select the Nursing Unit from the drop down list. 3. Select user name from the list of users. 4. Select the patient/room numbers from the table to the right by holding the CTRL key down and clicking appropriate patients with the mouse, they will be highlighted on the screen. 5. Press the Assign button. All patients are now assigned to the specific user. 6. Press the Unassign button to remove a user name after highlighting the patients. 7. Press the Save button to save the assignments that the user has made. The User has been assigned to the patient. Select Override Reasons. 2. Click new. 3. Enter a Override Reasons in the display text field. 4. Click the Save button. The Override Reasons list will display the information you entered only after you click the Save button. Script Setup Scripts allow iDOTs to be printed on forms or small signs which when scanned allow a certain computer programs, or script, to run. Scripts can create command iDOTs which, when scanned, will page someone, send e-mails, and other functions. Each script is created by IntelliDOT or your system administrator. Reports Refer to Generating Reports on page 4-1. intelliDOT iConnect System User Guide Confidential 3-41 DOT Organization Setup P/N TP0001-01 Rev. B DOT Organization Setup DOT Organization provides a way to add and organize groups of DOTs on certain iConnect reports. The DOT Organization provides a way to create groups of iDOTs for Override Reasons, Administration Sites, and Command DOTs. All groups can be configured to show differently on various reports. 1. Select DOT Organization The DOT Organization screen will appear. 2. Click New to create a new DOT group or click the DOT Organization group to edit. Note: To edit an existing DOT Organization group, select the group from the DOT Organization screen, then select the group members to remove or select the DOTs to add. Be sure to press SAVE to save all edits. 3. From the My DOT Group window, enter the name of the new DOT Group Name. 4. Select the type of DOTs to add (Override Reasons, Administration Site, or Command). 5. Select the DOTs to add and click on the Add button. The DOTs added appear in the Group Members window. 6. Click the Save button. The New DOT group name appears in the DOT Organization section. Note: To delete the DOT Organization list, select the group name and click the delete button. You will be prompted to save your information. B DOT Organization Setup iDOT Definitions When scanned iDOTs can prompt the iConnect System to carry out certain commands or actions. Refer to an example of a Medication Administration DOT sheet on the following page. This is a report that is hospital specific, but can be used to put all your DOT in one place. The DOT Keyboard report is another report which provides the basic DOTs that are used during each med pass. Refer to the table below. DOTs Cancel All Cancel Continue Omit Cancels all previous transactions. Description Cancels current transaction in relationship to medication when prompted by iConnect Handheld. Used to forward to the next action. Currently, user must keep pressing snooze unless med dose is truly omitted. When med given after this command, user may get a late warning. Respond with Med Held override reason. Logout Logs user out of the iConnect Handheld device. New Order Prep Appears if med is ordered and is not in the system yet.

B Medication Administration Record The The Medication Administration Record (MAR) information is located on the iConnect DOT Manager screen and allows you to view and print medication orders and medication administration transaction information for a patient. The MAR provides allergies, diagnosis, attending Physicians and order information for patients and may be used as the retrospective permanent chart ready hard-copy record of the MAR. Note: In the current released software version, you cannot print the whole nursing unit. This will be addressed in the next software release. Viewing your Medication Administration Report To view your Medication Administration Record, perform the following:
1. After logging into the iConnect DOT Manager, select Reports. The default report screen will appear. 2. From the Choose Report drop down menu, select Medication Administration Record. 3. From the Choose Printer drop down menu, select your applicable Printer. 4. Click the button to select a date. The Select Date window appears. 5. B Prospective MAR (by Patient) The Prospective MAR (by Patient) is a patient specific report designed for currently ordered medications for a unique patient. It can replace the handwritten notes many nurses write during report to keep track of their patients and important information about them. The Prospective MAR (by Patient) can serve as a reminder of medications due, and can be a convenient place to write notes about your patient. The Prospective MAR (by Patient) can be reprinted during your shift to update it with the latest medication information. Remember that leaving the Printer parameter blank will cause the report to be printed to the user's default printer when they scan the command iDOT for the report. B Chapter 5 Creating Command iDOTs Command iDOTs allow nurses to page someone, send an e-mail, print a document, and other tasks by simply scanning a iDOT. In order to place command iDOTs on reports, they must first be created. This chapter reviews the process for creating command iDOTs. For reporting-printing commands, if you always want the report to print on a specific printer when the iDOT is scanned, set up the printer in the command (the Order Change request for example). Once you have the commands setup, you put them on the Worksheet by adding the commands to the Worksheet's Command parameter. intelliDOT iConnect System User Guide Confidential 5-1 Command iDOT Prompt Setup P/N TP0001-01 Rev. B Command iDOT Prompt Setup The prompt setup is used to enter information that appears on your iConnect Handheld. The information you enter in the fields appear to the left in the blue box. B Tethering the iConnect Handheld Tethering is a way for the iConnect Handheld to directly interface with the iConnect System Manager. It is a fast efficient way of accurately entering bar code information into the iConnect System Manager without typing. Once tethered, you can scan a medication bar code and it appears in the formulary brand setup window of the Master Drug file. Tethering can be performed for the iConnect System Manager using the iConnect Handheld. To tether to the iConnect System Manager, perform the following:
1. Print out the latest DOT Keyboard. This keyboard will have a Tether iDOT on the report. 2. Login to the iConnect System Manager. 3. Logon the iConnect Handheld. Be sure to use the same login ID and password for the iConnect System Manager and the iConnect Handheld. To tether they must be the same. 4. On the iConnect System Manager, select the tether button from anywhere on the screen. Safety and Maintenance iConnect Handheld User Safety Information IMPORTANT: READ this before operating your iConnect Handheld For safe use of the iConnect Handheld always observe the following guidelines. This device complies with part 15 of the FCC rules. Operation is subject to the following two conditions: (1) This device may not cause harmful interference, and (2) this device must accept any interference received, including interference that may cause undesired operation. There are no user serviceable parts of the iConnect Handheld. Do not attempt to open the housing as this may damage the unit. -Consult the dealer or an experienced radio/TV technician for help. This unit incorporates a Class II laser in its bar code scanner. Laser light will be emitted for up to several seconds after the button labeled BAR CODE or III I II is pressed. The laser light is emitted from the side of the unit opposite the LCD and function buttons. Never direct the laser emitting side of the unit toward your own or anyone elses eyes. General Tips This device complies with part 15 of the FCC rules. Although the iConnect Handheld is water and dust resistant, do not expose it to rain or moisture for an extended period of time. Although the device is rugged, be careful not to drop the device or subject it to repetitive strong impact. Protect the iConnect Handheld from temperature extremes. Keep it away from heaters and other heat sources. Do not store the iConnect Handheld in a dusty, damp, or wet location. Cleaning the Device Use a soft lens cloth to clean the iConnect Handheld. If the surface of the screen becomes soiled, clean it with a soft cloth moistened with a diluted antibacterial soap solution. Batteries The iConnect Handheld uses one main battery. As long as the device is returned to the charger when not in use, the battery will stay operative and not need to be replaced. B Safety and Maintenance Battery Charging The iConnect Handheldcontains a rechargeable Li-ion battery with a charging safety circuit. When you first unpack the unit, the battery will likely be fully discharged. Place the unit into the iConnect charging station plugged into standard 120VAC power. It takes approximately 4 hours to fully charge the battery of the iConnect Handheld. Depending on usage, the battery should provide full function for about 12 hours. Wireless Networking The iConnect Handheld operates using 802.11b wireless networking capability. Your nursing unit must be equipped with wireless access points which allow the iConnect Handheld to communicate with the IntelliDOT Server computer and other iConnect Handheld. If you leave your nursing unit, the iConnect Handheld may loose its connection to the network and the IntelliDOT Server. Dont be alarmed, but know that your iConnect Handheld is off-line in these circumstances. iConnect Handheld Radiation Specifications The iConnect Handheld contains a laser device to implement linear bar code scanning. B Troubleshooting Unknown Patient Description When scanning the patient bar code, a match can not be made in the patient database Unknown Patient. Remedy Add the patient at the iConnect System Manager or check the interface or wait for the interface. Medication Can Not Be Found Description When scanning the medication, the med ID is not found in the formulary database Unknown Med. Remedy Contact the pharmacy or the IntelliDOT System Administrator. Time Error Description If a time error is received and the user knows that the order time is correct, the system time on the device may not be correct. Remedy Contact the IntelliDOT System Administrator. Device Does Not Turn On Description The login screen does not appear when the device is turned on. Remedy Insert a paper clip into the small hole on the backside of the iConnect Handheld. If the iConnect Handheld still does not operate, report the problem to the IntelliDOT System Administrator. Scanner Does Not Function Description The iConnect Handheld does not scan a bar code when you press the Bar Code button. Remedy Check to be sure the bar code is not defaced. Hold the scanner at a 30o-45o angle. Be sure the iConnect Handheld is within proper scanning range. If the iConnect Handheld stops emitting a laser beam while the Bar Code button is pressed, check the battery level. When the battery is low, the scanner shuts off before the low battery warning appears. Recharge the scanner and select another iConnect Handheld device. B Advance Prepared Items Case 3 - Prepared Item with multiple medications that is printed on the Prospective MAR This case describes the handling of a prepared item that is printed on the Prospective MAR. An iDOT will appear on the MAR to the left of the item. To prepare this dose:
1. Scan iDOT next to the medication order to be prepared on the Prospective MAR. In this example, the order is comprised of two medications <med name one> and <med name two>. The following screen will appear showing how many components are expected for scanning. Med Preparation

Scan Med ED iConnect Handheld screen 2. Scan the med bar codes for each of the ordered meds being prepared. 3. Verify that the correct medication is displayed, then press OK. If the medication does not require entry of the dose amount, proceed to step 6. If entry of the dose amount is required, the following screen will appear:

Enter Dose ED iConnect Handheld screen 4. Enter the dose amount for this ingredient using the numbered iDOTs on the Medication Administration DOT sheet. The screen will display your entry Multi -Dose Vial 10 ML Accept OK or Enter Dose ED iConnect Handheld screen 5. If you entered the incorrect dose, scan the Back iDOT on the Medication Administration DOT sheet to back up one space. Scan the # iDOT clear the previous entry and reenter the correct amount. 6. Press the OK button to accept the entry. The following screen will appear:

B Communicating Directly with Pharmacy using the iConnect Handheld The iConnect System allows the nurse to quickly and easily communicate with the pharmacy. Specific messages are defined by the hospital's iConnect system administrator. An example of a useful application of this feature is to request a change in administration times for a specific medication order. (Having the pharmacy change the dosing times for an order in the Pharmacy Information System is the only way to adjust the iConnect dosing schedule for a medication order.) To allow a message from the nurse to pharmacy to request a time change, an iDOT for the message "Change Med Time" must be created by the system manager. Message iDOTs can appear on Prospectiv MARs. Messages are received on an iConnect printer located in the pharmacy department. The following outlines the steps for sending a Change Med Time request to pharmacy:
1. Scan the Change Med Time iDOT on the Prospective MAR. 2. Enter new scheduled time(s) using the numeric iDOTs on the Medication Administration DOT sheet. 3. B Pharmacy Inventory Verification Using the iConnect Handheld Pharmacy Inventory Verification Using the iConnect Handheld Once the iConnect medication formulary is complete with one or more valid bar codes for each item in place, Pharmacy needs a method for quickly verifying that a product manufacturer bar code is present in the formulary. This is important to avoid stocking and dispensing of items that are unknown to intelliDOT and therefore cannot be scanned at the time of administration. The iConnect Handheld makes this verification extremely fast and easy for Pharmacy. To determine whether a manufacturer's bar-coded item exists in the iConnect formulary and is ready for use:

B Printer Setup The IntelliDOT system allows all printers on your hospital network to work with IntelliDOT. Adding, Editing, and Deleting Printers To add the Printers, perform the following:
1. Click Configuration on your IntelliDOT DOT Manager. The Configuration (Hospital Address screen will appear). 2. Click Printer Setup. The Printer Setup window appears. 3. To add a printer, click New and enter the Friendly Name and Device Name. Note: The Friendly Name is what you will call the printer. The Device Name is the name of the Device the network administrator has assigned to the device. 4. To search for a specific printer, select the drop down box (Friendly Name or Device Name). 5. Enter the printer name and click Search. The specified printer appears and is highlighted. 6. Click Save to save all your information. Note: To edit the Printers, select the printer and edit the fields. To delete the Printers, select the printer, click delete and click OK. B Configuration Setup Shift Setup Shift Setup is used to enter and store all nurse shift start and end times used in all areas of the hospital. Typically, your system administrator enters all of this information. All shifts schedules worked on all units should be entered. This is not nursing unit specific. To add the Nursing Shifts, perform the following:
1. Click Configuration on your IntelliDOT DOT Manager. 2. Click Shift Setup. The Shift Setup window appears. 3. To add a shift, click New. 4. Enter the Shift Name. 5. Enter the Shift Start and Shift End. Note: The Schedule for the nurses is based on start times of schedules. If your schedule is 12 hours, the shift would start at 7:00am and end at 7:00pm. The new shifts will appear in the top of the window as you have entered them. 6. To search for a specific shift, select the drop down box (Shift Name, Shift Start, or Shift End). 7. Enter the desired information and click Search. The specified shift will appear and is highlighted. 8. Click Save to save all your information. Note: To edit the Shift, select the shift and edit the fields. To delete the shift, select the printer, click delete and click OK. You will be prompted to save your information. intelliDOT iConnect System User Guide Confidential 3-5 Formulary Setup P/N TP0001-01 Rev. B Formulary Setup As part of the iConnect System, the hospital pharmacy electronically transmits the Hospital Formulary to the iConnect server database. The iConnect System maintains a copy of the pharmacy formulary and stores bar code data for all pharmacy formulary line items. Each new item received by pharmacy must be checked to assure it is in the iConnect database. This process is done using the iConnect Handheld in verify mode. Refer to Pharmacy Inventory Verification Using the iConnect Handheld on page 2-49. Items which are not found in the iConnect database must be entered, and bar code information scanned and stored. Items received without bar codes printed on them must be packaged or labeled with a bar code. Each formulary line item must be configured for a variety of clinical settings, parameters and warnings. The following configuration settings are explained. The clinical settings and warning parameters are set for each formulary drug item. Clinical Warnings - Setup by Pharmacy Clinical Prompts - Setup by System Administrator Master Drug File - Setup by Pharmacy and System Administrator (Clinical Prompts) Clinical Warnings Clinical warnings displayed during medication administration are entered here. The pharmacy must set up the clinical warnings prior to assigning them to the formulary items. Warnings must first be entered here. Then they are available to be linked to individual formulary line items. Adding, Editing and Deleting Clinical Warnings 1. To add Clinical Warnings, click New. The Edit Clinical Warnings screen with empty fields will appear. 3-6 Confidential intelliDOT iConnect System User Guide P/N TP0001-01 Rev. B Formulary Setup 2. Enter the clinical warning in line 1 and line 1b. Note: You can enter up to 16 characters each in the line 1 and line 1b (No more than 32 for both). The Clinical Warning will display in the display box to the right of the fields as it will appear on the iConnect Handheld screen. When line 1 reaches 16 characters, additional text is added in line 1b which then appears. Line 1 will alternately flash with line 1b when displayed during med administration. 3. Check Disabled System Wide if you do not want this warning to appear on any iConnect Handheld. The flag enables the hospital to turn warnings on or off without needing to edit every formulary item that those warnings are associated with. When this feature is checked, the warnings will not appear for the displayed medication on the iConnect Handheld. 4. To search for a specific clinical warning, select the drop down box (Line 1, Line B, or Disabled), enter the desired name and click Search. The specified warning will appear and is highlighted. 5. Click Save. The Clinical Warning appears in the Clinical Warning List. Note: To edit the Clinical warning, select the warning and edit the fields. To delete the Clinical warning, select the warning, click delete and click OK. Enter the clinical prompts in line 1 and line 1b as you would like it to display on the top line of the iConnect Handheld. Note: You can enter up to 16 characters each on line 1 and line 1b (No more than 32 for both). The Clinical Prompt information will display in the Sample Display box to the right of the fields. Line 1 and line1b alternately flash to provide the nurse with the clinical prompt information during med administration. 4. When this feature is checked, this warning will not appear on the iConnect Handheld for any medication system-wide. It is only checked if nursing does not want it displayed on the iConnect Handheld every time the medication is administered. 7. From the drop-down menu, select the applicable Response Category. 8. To search for a Clinical Prompt, select the drop down box (Line 1, Response Category, or Disabled), enter the desired name and click Search. The specified prompt will appear and is highlighted. 9. Click Save. The Clinical Warning appears in the Clinical Warning List. Note: To edit the Clinical Prompt, select the prompt and edit the fields. Formulary Configuration Once the formulary download is complete, configuration is completed as follows:
1. Click on Formulary on your iConnect System Manager. Clinical Warnings, Clinical Prompts, and Master Drug File will appear. 2. Click on Master Drug File. The Formulary screen will appear. 3. From the Formulary Listing, select a med. Note: The hospital pharmacy must always be checking the formulary to make sure the bar code for each medication is correct. Meds without bar codes cannot be administered at the bedside and the pharmacist will need to maintain this information. The iConnect system requires all medications to be barcoded. 4. B Formulary Setup The Formulary Brand Setup for the medication window appears. 5. Tether the iConnect Handheld. Refer to Tethering the iConnect Handheld on page 6-2. 6. Scan your medication bar code. The bar code information will display in the bar code field. If the Brand Name is not present, enter the Brand Name. 7. 8. Enter the NDC number. Note: The NDC number is not always the same as the bar code number. You may have packaged this medication via your medication packaging system. If so, select Hospital Packed from the drop down menu instead of NDC. If you did hospital package the medication, typically this number is a non-NDC number. 9. Click the OK button to save this information. The following screen references steps 8 through 16. 10. Check the Prepared Item box if this medication requires advance preparation by the nurse prior to administration away from the bedside. Refer to Advance Prepared Items on page 2-25. 11. Check the Preparation Requires Witness box if the medication requires a witness. Refer to Case 2 - Prepared Item that is printed on the Prospective MAR and requiring a witness on page 2-28. 12. Check the Prompt for dose entry box if the dosage form allows the nurse to administer less than the entire contents. 13. Check the Show route warning box if a route warning needs to appear on the iConnect Handheld. B Note: Route warning should be used for medications where more than one route of administration is possible. For example, an injectable med may be given IM, SQ or IV. The route warning will display the ordered route at medication administration. If a med has a route warning of Give IV the checked box provides a way the prompt to appear on the iConnect Handheld. 14. Check the Disable Dose Due Reminders box if you do not want the nurse to be reminded when this medication is due. 15. Configure for Clinical Prompts by selecting from entered choices. Note: Clinical Prompts must be added before using this screen. Refer to Clinical Prompts on page 3-8. Clinical Warnings must be added before using this screen. Refer to Clinical Warnings on page 3-6. 16. Click the Add button to add a clinical prompt or clinical warning. A list of prompts appear. 17. Select the prompts and the press the OK button. The clinical warning or clinical prompt appear if for example: a med has a clinical warning of Do Not Crush or a clinical prompt of Check Blood Pressure. This checked box provides a way for the prompt to appear on the iConnect Handheld during medication administration. 18. To remove the prompts, select the prompt and click the Remove button. The prompts will be removed from the Clinical Warning and Prompt windows. 19. Click Save to save all information. B Nursing Unit Setup Nursing Unit Setup Each nursing unit using the iConnect System must be entered here, along with all room numbers located on the unit. Each nursing unit must be separately entered and configured. The Auto Fill section of the nursing unit is a time saving feature that allows you to automatically enter room numbers within a certain range for that area and add and remove applicable rooms. Adding, Editing, and Deleting Nursing Units To add a new nursing unit, perform the following:
1. Select Nursing Unit. The Nursing Unit screen will appear. 2. Click New and enter a Name for the Nursing Unit. 3. Enter the Reminder Delay %. Note: The delay is the % of the lag time before a med due reminder is shown to the nurse. For instance, if the lag time is 1 hour and the reminder delay % is 50%, then at 30 minutes after the due time the nurse will receive a med due reminder. If the setting is 25%, then at 15 minutes past the due time, the nurse will receive a reminder. If set to 100%, the nurse will never see a Med Due reminder, but will just receive a Med Late reminder after the complete lag time has past. To enter Enter the bed or room numbers, click the Auto Fill button. The Room Auto Fill window will appear. 5. Enter the Start Room, End Room and Beds (comma separated) and click OK. Autofill will fill in sequential room numbers entered. If room numbers are not sequential, manual entry must be used. Note: Route warning should be used for medications where more than one route of administration is possible. For example, an injectable med may be given IM, SQ or IV. The route warning will display the ordered route at medication administration. 6. Click Save to save all information. If you do not click the save button, you will be prompted to do so after exiting the iConnect System Manager screen. Note: To edit the Nursing Unit, select the Nursing Unit and edit the fields. To delete the Nursing Unit, select the Nursing Unit, click delete and click OK. B Sig Code Setup Sig Code Setup A "sig" is the physician's directions for when and how often an ordered med is to be given. Examples of sigs are BID (twice a day), Q4H (every four hours), and PRN (as needed). The "Sig Code" listings must be stored in the iConnect system to be recognized when they are received from the pharmacy system. Each sig code that is used for order entry purposes in the pharmacy information system must be defined in IntelliDOT. Adding, Editing, and Deleting Sig Codes To add sig codes, perform the following:
1. Select Sig Codes. The Sig Codes screen will appear. 2. Click New and enter the name of the sig code. The cursor will appear in the name field. 3. Click in the Display Text field and enter the Display Text information. 4. Check the May Repeat box to set the sig code to repeat the administration. May repeat defines the number of actions that a medication can be administed. After the total number of actions has been met, the order is ended. 5. Check the Every button, enter the desired number (1-7), and select the Day/
Week/Month to set the sig code up for specific Days, Weeks, or Months. 6. Check the Specific days of the week button and check the specific days
(Sun-Sat) to set the sig code up for specific days. 7. Check the Every button, enter the desired number (1-24), and select hour/
minute/second to set the sig code up for specific hours. 8. B Sig Code Setup Adding, Editing, and Deleting a One Time Order A one time order is an order which is only to be administered once. The medication time for a one time order may be specifically assigned (i.e. administer at 08:00), or it may be ordered as a "now" order. "Now" one time orders are expected to be given at the time the order is signed off and therefore may be noted at any time. In addition, a one time order may be written as a pre-op order, in which case, the time will depend on a surgery or procedure schedule. One time orders result in a single event or action and after the one time dose is administered the order is ended. To add a one time order sig code, perform the following:
1. Select Sig Codes. The Sig Codes screen will appear. 2. Click New and enter the name of the sig code. The cursor will appear in the name field. 3. Click in the Display Text field and enter the Display Text information. 4. Check the One Time Order box if an order is only to be administered once. 5. Select the Discrete Dosing Times button. B Sig Code Setup Adding, Editing, and Deleting a PRN A PRN order is an order which can be administered on an "as needed" or recurring basis. A PRN order may have timeframes such as "give every 3-4 hours PRN"
which indicates that the patient is not to exceed a dosing schedule of 3 hour intervals. A PRN order might have a end date/time established, which restricts the nurse in administering any doses beyond the end date of the order. A "one time PRN" order enables the nurse to give the medication one time, but not at any specifically timed schedule. After the dose is administerd, the order is ended. To add a PRN sig code, perform the following:
1. Select Sig Codes. The Sig Codes screen will appear. 2. Click New and enter the name of the PRN sig code. The cursor will appear in the name field. 3. Click in the Display Text field and enter the Display Text information. 4. Check the PRN box to administer a medication on an "as needed" or recurring basisis. 5. B Role Setup Role Setup Role Setup allows the system administrator to identify classes of user and privileges of users in the iConnect system. Roles are created for specific pharmacy and nursing functions. Each role can have different privilege depending on your hospital security levels. The IntelliDOT system gives you the ability to have as many roles as you desire. 1. Select Role Setup. The Role Setup screen will appear. 2. Enter a role name. 3. Enter a set of privileges for each role. Each privilege for a role is different. Depending on your system privileges within the hospital, you may only see certain functions in your iConnect System Manager Tree (left side of screen). Example: the pharmacist will have all privileges, the staff nurse will have viewing privileges. The new privileges that you can use to tailor your users' access to the system are listed below.
